SUMMARY OF POSITION:

This position is for a compassionate and skilled Maternal Health Social worker to support pregnant and postpartum patients and their families by addressing psychosocial needs, coordinating care, and promoting healthy outcomes for both mother and baby. Responsibilities will include crisis intervention, emotional support, counseling related to pregnancy, birth, and postpartum adjustment, screening for mood/anxiety disorders, substance abuse, domestic violence, child abuse, and various SDoH. This staff member will collaborate with interdisciplinary teams including providers, nurses, lactation consultants, risk management, legal, and case managers. This position will develop and implement safe discharge plans for mother and newborn. You will coordinate referrals to community resources, including home visits, behavioral health, and social services. Will facilitate mandated reporting when necessary. Will educate patients and families on available resources and support systems. Will maintain timely and accurate documentation in the EMR. 

MINIMUM QUALIFICATION(S):

  • Masters of Social Work degree required.
  • AHA Basic Life Support (BLS) – required prior to the end of the orientation period 

PREFERRED QUALIFICATION(S):

  • Previous experience with the FBC/NICU/Peds population and/or Children’s Youth Services is preferred.
