SUMMARY OF POSITION:
Responsible to coordinate interdisciplinary professional care of Structural Heart disease patients as determined by individualized plans of care and clinical pathways. Functions include but not limited to RN triage, patient phone calls and follow up, care coordination, patient intake, insurance authorization, and medical record reviews. Attendance and participation in the weekly multidisciplinary patient meetings are integral to act as ambassador and support for patients.
This is an in-person clinical cardiac position which requires a fundamental knowledge of Cardiology, with preference to Structural Heart disease.
MINIMUM QUALIFICATION(S):
- Bachelor's degree-Registered Nurse
- Two (2) years nursing experience
- Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
- AHA BLS
PREFERRED QUALIFICATION(S):
- Experience as clinical case manager/attending nurse preferred.
- Experience in general Cardiology, Structural Heart, Interventional Cardiology preferred.
- Experience in Inpatient and Outpatient operations preferred.
