SUMMARY OF POSITION:

The Social Worker II provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family caregivers. Provide patients with information and counseling and make referrals for other services. May also provide case and care management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. 

Responsible for collaborating with the members of the health care team to provide comprehensive care to improve patient's social and behavioral concerns that impact overall wellness. This position will be an expert in their field. They will act as a mentor to other Social Worker staff as well as expectations of serving as a leader on the team and with other disciplines. Facilitate the coordination of appropriate patient centric and cost-effective services that are consistent with the patient's plan of care to help achieve his/her optimal level of independence and enhance the quality of life. For the purposes of the document the primary population of focus is pediatric hematology/ oncology patients. healthcare. 

MINIMUM QUALIFICATION(S):

  • Master's degree
  • Two (2) years social work experience
  • Completed the probationary period for their current role, must be an existing employee of Care Transitions
  • Accredited Case Manager
  • Maintain certification as a Certified Case Manager, either CCM or ACM. NOTE: Case manager certification is required if candidate is not a Licensed Clinical Social Worker (LCSW).
  • Maintain membership in one professional case management organization (ACMA, ACCM, or CMSA). LCSW’s may substitute membership in the National Association of Social Workers (NASW).
