SUMMARY OF POSITION:

Join Our Dedicated Team as an Ambulatory Care Anticoagulation Clinic Pharmacist at Penn State Health!

Become a vital part of our mission to deliver exceptional patient care as a full-time Ambulatory Care Anticoagulation Clinic Pharmacist at Penn State Health. This role provides a unique opportunity to directly impact patient outcomes within our adult patient population, working with our pharmacists within a collaborative practice agreement. You will leverage your pharmacist and people skills to optimize safe and effective care of our patients taking warfarin.

Your primary responsibility will be the provision of direct care to patients, both in-person and via telephone communication, providing education and ongoing anticoagulation management.

Other facets of the position include:

  • Training: Initial & Ongoing training in Anticoagulation Management including interpersonal communication. Opportunities to complete USI certificate course and, in future, anticoagulation certification.
  • Education of future health care providers: Precept students and PGY1 pharmacy residents. Expose anticoagulation management to PGY1 internal medicine medical residents.
  • Quality Improvement: Update and develop system-wide anticoagulation protocols and initiatives to enhance patient care.

This full-time position currently offers a balanced work-life schedule:

  • Monday - Friday, between 7:30 am - 5 pm.
  • Saturdays in a multi-person rotation.
  • No evenings, Sundays, or holidays.

MINIMUM QUALIFICATION(S):

  • Bachelors of Science degree in Pharmacy or Doctorate Degree in Pharmacy required.
  • ASHP-Accredited PGY-1 Pharmacy residency or 3 years of relevant clinical pharmacy experience required.
  • Active and valid PA licensure or eligibility as a Pharmacist required.
