  2. Provider Portal Questionnaire - a short questionnaire to help the Health Information Management team determine the best access to meet your practice site’s needs.
  3. Provider List - outlines all providers (MD/DO/CRNP/PA/APCs) who are required to have their patients available on the Provider Portal. If your providers see patients at multiple locations, please add and identify those locations. If your practice utilizes direct secure email addresses, please also provide the provider’s direct secure email address. Direct secure email addresses allow us to also securely send you Patient Health Information (PHI). If you currently do not have direct secure email addresses and are interested, see your certified EHR software vendor. For more information, see https://www.directtrust.org/what-we-do, Direct Secure Messaging section.
  4. Provider Portal Technical Checklist - this checklist is to be completed by the practice site’s Information Systems department or Practice Site Manager. This checklist outlines the practice site’s Information Systems environment and will give our staff a clear idea of the compatibility with the Provider Portal.
