How to transfer your medical records

Choose one of four options to transfer your medical records:

  • Download the Release of Records form. Sign and send it to the Penn State Health practice of your choice.
  • Visit the Penn State Health practice of your choice. While there, sign and submit the Release of Records form.
  • Ask the Penn State Health practice of your choice to mail the Release of Records form to you.
  • Complete the transfer of your medical records with your previous provider.
