Container Whole Blood Required

5 mL green (Na Heparin) top tube for patient and 5 mL green (Na Heparin) top tube for control blood. Collect specimen Mondays through Thursdays only. Ordering physician name and phone number are required. SPECIMEN REQUIREMENTS: Two separate specimens (a whole blood Na Heparin specimen and a whole blood Na Heparin control from an unrelated, healthy donor) are required.
