Social Worker/ Therapist (PRN) - Behavioral Health Inpatient Acute Care
SUMMARY OF POSITION:
The Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family caregivers. The Social Worker provides patients with information and counseling, and make referrals for other services. They counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life. The Social Worker plans discharge from care facility to home or other care facility. They organize support groups or counsels family members to assist them in understanding, dealing with, and supporting the client or patient. The Social Worker may also provide case and care management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
MINIMUM QUALIFICATION(S):
Master's Degree in Social Work required
Must obtain ACM within two (2) years of eligibility or current CCM
