RN Registered Nurse Navigator – Family Birthing Center - Penn State Health St. Joseph (Reading, PA)
The RN Registered Nurse Navigator is responsible for coordinating the individualized care continuum for patients and for achieving quality, cost-effective and patient-focused outcomes by serving as an advocate, educator, consultant, leader and community ambassador for this specific patient population in accordance with hospital standards/guidelines, applicable statutory requirements and accepted clinical practice for the clinical nurse specialist profession. The RN Registered Nurse Navigator is a consistent care coordinator throughout the continuum of care and is able to assess the physical, psychological and social needs of the patient. Work is strongly focused on: 1) guiding the patient/family through the complex healthcare system, working to eliminate barriers that might otherwise adversely impact patient care/outcomes; and 2) helping to coordinate the efforts of the medical team to facilitate continuity of care and enhance patient satisfaction. Work requires strong nursing expertise and an understanding of navigation concepts, patient care standards/practices and applicable regulatory requirements specific to the assigned specialty area. An incumbent exercises a high degree of discretion/independent judgment and applies advanced skills obtained through extensive clinical preparation to ease the burden on patients/families while improving patient outcomes through monitoring and education. Situations encountered entail the application of sound critical thinking skills in the face of highly stressful situations impacting the patient/family and involving difficult decision-making.
Bachelor's degree in Nursing (BSN) from an accredited college or university
Current BLS Certification
Five years of related professional nursing work experience
Master’s degree in Nursing (MSN) or related field is strongly preferred
Previous navigator work experience is strongly preferred
Minimum of two (2) years Labor & Delivery, Maternity, Postpartum RN Registered Nurse experience preferred
