SUMMARY OF POSITION:
Responsible for supporting the Penn State Health Patient Logistics department in the day to day operations of the health system hospital transfer center and Hershey Medical Center patient placement functions, ensuring exceptional customer service and quality care. Also responsible for supporting the Transfer Center Liaison and Patient Placement Coordinator roles to ensure adequate staffing and call acceptance at all times. Real time coordination of health system capacity management and referral acceptance to the most appropriate PSH entity. Manages real-time quality assurance reviews, holds staff accountable to high customer service standards as primary contact for all PSH transfer referrals, and escalates critical clinical issues related to hospital transfers and patient placement in real time. Responsible for real-time communication of current capacity status and Health System bed availability to Patient Logistics leadership for dissemination to all PSH operational leaders and senior leadership. Coordinates, prioritizes, and optimizes patient flow using knowledge of clinical and operational activities throughout the hospital network.
ESSENTIAL DUTIES: The percentage of time spent performing essential functions is 95%. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
- Acts as first escalation point for the 24/7 call center managing all health system referral calls and patient placement activities.
- Actively monitors TeleTracking patient placement flow board, the Capacity Management Board, and Transfer Center board to identify barriers to patient acceptance and flow.
- Maintains call Ready status as first call escalation to mitigate caller concerns at time of issue.
- Investigates and mitigates any immediate barrier to patient transfer acceptance to ensure the propagation of HMC’s reputation in the community.
- Investigates and mitigates any immediate barrier to patient placement within HMC to ensure patient and staff safety is maintained at all times.
- Immediately escalates any barriers that cannot be resolved within thirty minutes of onset.
- Performs all Transfer Center Liaison functions of referral call management for PSH health system entities and patient chart review for HMC.
- Coordinate the facilitation of external facility/clinic transfers related to beds, estimated time of arrival, and maintaining guidelines for compliance and regulatory requirements, maximizing reimbursement and minimizing out-of-pocket expenses for patients.
- Complete patient registration to include demographic, clinical, insurance and regulatory information.
- Accurately enter all information into the registration system selecting the correct medical record number, the appropriate visit type and creating or selecting the appropriate Occasion of Service number as applicable.
- Accurately target and assign beds within the Patient Flow Software for all admissions and transfers.
- Accepts phone calls from numerous stakeholders, both internal and external that includes but is not limited to physicians, nurses, nursing leadership, housekeeping, transport etc.
- Performs and understands all Patient Flow Coordinator functions in order to understand and ensure consistency across the entire department.
- Completes standardized quality and assurance reviews for all work completed by the Transfer Center Liaisons and Patient Placement Coordinators.
- Facilitate downtime functions to ensure proper back loading of information into all systems. Utilize working knowledge of REACH, Cerner, FirstNet, the TeleTracking Suite, and Transfer Center.
MINIMUM QUALIFICATION(S):
- Bachelor’s degree in Nursing required.
- Five (5) years Critical Care Nursing experience required.
- Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
