Our exemplary team of Inpatient Social Workers are essential liaisons between patient, family, physicians, community and organizational resources to promote the coordination of a comprehensive treatment plan. They are responsible for providing complete psychosocial assessments, crisis intervention, individual, family, group, bereavement and end of life counseling, and timely and appropriate discharge planning based on known or anticipated needs.

Our Inpatient Social Workers collaborate with physicians, staff nurses, and care coordinators to determine patient care needs, readiness for discharge, and the need for home-health services or other community-based resources. This individual will play a significant role in making connections and referrals to the health agencies, mental health facilities, counseling services, and other social and governmental departments that would be the best resource in ensuring patient needs are met.

As a member of our team, you are eligible to qualify for our clinical ladder program and have unlimited access to the American Case management COMPASS training as well as CEU’s.


Masters of Social Work degree from a Council of Social Work Education school of social work with 1 to 2 years effective experience required


Ability to establish and maintain a collaborative relationship with internal and external health professionals

Excellent written and oral communication skills

Ability to use problem solving techniques to achieve desired results and effective change

Ability to function effectively in a dynamic and rapidly changing environment

Ability to organize, prioritize, and time manage responsibilities

Basic computer skills with effective keyboarding abilities


Minimum of two years of acute care case management/social work experience

Knowledge of PC based computer software (Cerner, Navihealth/Curaspan)

Current social work licensure in Pennsylvania

Certification in case management from a nationally recognized organization
