This job is responsible for providing professional nutrition services and assessments of patient’s nutritional status to include needs assessment, planning, counseling and education. Responsible for providing nutritional education to the medical and general community. Key contacts include: physicians to identify appropriate patient nutrition plans; Department director to participate in staff development; and patients and families to educate and counsel on nutrition principles, diet regimens, and food selections and preparations based on diagnosis. Responsible for providing comprehensive nutrition assessments and care planning for patients. Implements business practices in order to uphold Company mission and values.
Drive and Dependability
Assesses, plans and implements the delivery of nutritional care to patients in collaboration with the inter-disciplinary team.
Assesses both inpatient and outpatients.
Develops, maintains and modifies patient’s nutrition plan.
Collaborates and communicates effectively with the Inter Disciplinary Team.
Communicates patient’s nutritional needs to the diet office.
Provides nutritional counseling and education to individuals and/or groups.
Develops, organizes and offers classes on nutritional education within the facility and the community at large.
Documents nutrition care in accordance with department-specific standards.
Collaborates and communicates with culinary and clinical departments on established processes, special diets and menu requests.
Stays current with most recent nutrition-related trends, research, policy, certifications and standards of practice.
Probes potential problems and apprised manager of status on resolution of problems or issues.
Ensures compliance with all federal, state and local regulations as well as company policies and procedures. (Example, HIPPA/Privacy Act, quality assurance, safety, operations and personnel)·
Implements quality improvements and communicates then to other departments.
Provides education to the Medical Staff as requested.·
Submits patient charges and billing information.
Organize outpatient schedules and work with Central Scheduling and Provider.
Performs related duties as required.
Bachelor’s degree is required with a major in nutrition and dietetics preferred.
Completion of internship.
One (1) year recent experience in clinical dietetics in a hospital setting.
Outpatient experience preferred
Licensure/Certification by the Commission on Dietetic Registration/Academy of Nutrition and Dietetics Required
Licensure by the State of Pennsylvania as a Licensed Dietitian/Nutritionist (LDN) is required.
Membership in the Academy of Nutrition and Dietetics
