The Clinical Pharmacist at Penn State Health is responsible for providing the prospective evaluation and monitoring of drug therapy and make patient specific recommendations to improve pharmacotherapy outcome for attributed patients.


This position will work within a collaborative care relationship with providers to provide patients with direct access to medication therapy management and enhanced quality of care.  The Clinical Pharmacist will be located in the Academic Support Building on the Milton S. Hershey Medical Center campus. 




Focus on clinical activities in order to impact certain quality outcomes such as STARS/HEDIS measures, as well as reduce clinical outcomes such as ER visits, inpatient stays, and readmissions.    


Will review patient charts based off of patient lists showing noncompliance with medication adherence


Will partner with physician and Quality Team, and work with patient to address barriers (via telephone, or portal message).


Will deliver direct patient drug consultation and support through Medication Therapy Management in such areas as proper drug utilization, adherence, demonstrating correct drug administration and education, identifying potential drug-drug interactions, etc. 

Some opportunity for face-to-face review with patient if practice site has space.


Will recommend to the physician alternative medications based on collaboration with payors and best practices.


Will require documentation within the Electronic Medical Record (EMR) and communication with the care team


Provide inservice education to physicians, nurses, and other practitioners on a range of pharmacotherapy topics.


Review and interpret medical literature and apply conclusions to specific patient situations to develop the most practical and evidence-based recommendation to optimize drug therapy.




Bachelors of Science degree in Pharmacy or Doctorate Degree in Pharmacy required.

ASHP-Accredited PGY-1 Pharmacy residency or 3 years of relevant clinical pharmacy experience required.


Active and valid PA licensure or eligibility as a Pharmacist required.




Must be comfortable working collaboratively as part of a care team

Ability to prioritize and organize daily activities to meet patient care needs required.

Effective verbal communication skills, time management skills and ability to manage multiple priorities required.

Ability to analyze clinical data and perform calculations.

Computer and Microsoft Office skills required.

Extensive knowledge of pharmacotherapy required.

Attention to detail and critical thinking skills required.


Effective mentoring and precepting abilities required.




Hospital or Outpatient Clinic experience preferred


Telemanagement experience preferred


Experience working with Quality Outcomes (STARS measures, HEDIS metrics etc.) preferred
